You should complete the Cornell course and receive a letter of course completion by November 20, 2009 in order to be enrolled in either the November 23rd or 24th, 2009 class.

Course fee is $60 (includes AFDO certificate, refreshments and course handouts). Checks or money orders only.  No credit cards are accepted.  Students may email their letter of course completion issued by Cornell University by November 20, 2009 or bring a printed copy to the course.

Students must bring a copy of the HACCP Training Curriculum and Hazards Guide.  The books may be ordered for $40 plus postage (SGR 120 and SGR 121) from the University of Florida (http://ifasbooks.ufl.edu/merchant2/merchant.mv?Screen=CTGY&Store_Code=IFASBOOKS&Category_Code=SGBK), or downloaded to a laptop or printed from http://seafoodhaccp.cornell.edu/manuals_pdf.html .

You may bring your own lunch or purchase a box lunch at the time of check-in for $12.50 (exact change is required).

Please bring your own pen, paper and highlighting marker.  You should review the 7 principles of HACCP and the 8 sanitation control procedure conditions prior to the course.

You are highly encouraged to review the study guide prior to attending the course.

A government issued photo identification (i.e., driver's license or passport) is required.

You are responsible for your own lodging.  The Hilton Irvine/Orange County Airport Hotel provides complimentary shuttle service to and from the FDA office in Irvine:

Enrollment is limited.   First come, first served.
